North Country Hospital
**Statement of Purpose:**
This is a full-time outpatient pediatric position in a four-provider hospital owned pediatric practice (3 MD, 1 APP) with excellent physician support and mentoring. The candidate will see outpatients for well care, acute care and chronic care maintenance 4 days per week and one Saturday morning per month and will cover first call (outpatient only) with physician backup 2 nights per week.
North Country Pediatrics is a RHC certified Medical Home that offers wrap around care to our patients including WIC in the Medical Home, dietician, asthma trainer, DULCE worker, case manager and embedded therapist.
We are looking for a well-trained APP, ideally with pediatric experience, who works well in a team, is dedicated to high quality patient care and looking to grow with the position as we participate in a lot of QI projects.

**Job Requirements:**
Completion of an accredited program for nurse practitioners or physician assistants. Appropriate licensing/registration. Preferably 3-5 years of experience, with emphasis on Pediatrics. Ability to work collaboratively with physicians, other health care providers, and parents. Practice guidelines will be reviewed, mutually agreed upon, and signed annually by the APRN or PA and the collaborating physician and placed on file in the workplace.
