**Description**
Gifford has an opportunity for a Nurse Practitioner to join our well established, Diabetes Clinic. Working closely with the diabetes educator, this clinician will see patients in two outpatient clinics providing support, education and CGM technology for our patients. This position often acts in a consultative, supportive manner with other Gifford clinicians. Gifford embraces a team approach to patient care, supported by collaborative relationships throughout the organization.

Minimum 2-4 years of experience in a clinical setting preferred. This is an exempt position that includes benefits.
